What We Do In PE
Medical Excuses
 
  • A note from a parent or doctor is required in order for a student to be excused from participating in class activities.

 

  • A parent's note is valid for THREE (3) days only, this includes weekends. The note must include the following:
    • Student's name
    • Specific reason for excuse
    • Parent signature with date
    • Daytime phone number of parent
  • After THREE (3) days, a doctor's note is required.

 

  • A parent or doctor's note DOES NOT excuse a student from the following:
    • Attending class
    • Dressing in PE uniform (unless medical condition prohibits)
    • Assisting teacher in class activities

 

  • Students who have a long-term or permanent health condition, such as asthma, that limits normal class participation, must have a modified PE form completed by a doctor and filed in the nurse's office.

 

  •  Students on a long-term medical excuse (over 1 week) will be given a written assignment to be completed for their PE grade. Must see MS. ALONZO for this assignment.
