| Medical Excuses A parent's note is valid for THREE (3) days only, this includes weekends. The note must include the following: Student's name Specific reason for excuse Parent signature with date Daytime phone number of parent
After THREE (3) days, a doctor's note is required.
Students who have a long-term or permanent health condition, such as asthma, that limits normal class participation, must have a modified PE form completed by a doctor and filed in the nurse's office.
